Everyday Care With a Long-Term View

From establishing primary care to returning for follow-up, your visit is a chance to look at both today’s concerns and your longer-term health. We can discuss symptoms, review changes since your last appointment, and help you understand your options.

Your care plan should make sense in your everyday life. We consider your medical history, current medicines, work and family responsibilities, and personal health goals when discussing treatment and follow-up. When a concern needs additional evaluation or specialty care, we can discuss appropriate testing or referral options.

Three Priorities for Adult Health

Primary Care connects prevention, care for new symptoms, and ongoing support for chronic conditions. The focus of each visit depends on what you need, whether that is a wellness discussion, a recent change in your health, or a review of an existing treatment plan.

Preventive Care and Wellness

Discuss health goals and which preventive screenings or immunizations may be appropriate for your age, history, and circumstances.

Care for New Concerns

Bring questions about new symptoms or short-term illnesses. We assess your concerns and discuss next steps, including follow-up or additional evaluation when needed.

Chronic Condition Follow-Up

Review ongoing concerns such as high blood pressure or diabetes, your medicines, and any changes that may call for adjustments to your care plan.

Choose the Right Starting Point

A new illness and an ongoing condition may call for different conversations. Explore our acute illness care and chronic disease care pages, then ask the clinic which appointment fits your concern. Bring a medicine list and the questions you most want answered.
